Northgate and Sollis provide new system to improve patient care for Gloucestershire and Swindon CCGs

Northgate Public Services (Northgate) and Sollis today announced a successful deployment of  the Sollis Clarity Patients solution with integrated Adjusted Clinical Groups (ACGs) across Gloucestershire Clinical Commissioning Group (CCG) and Swindon CCG, as a hosted service to over 100 GP practices across the areas.

Northgate and Sollis will support Gloucestershire and Swindon in their strategic objective of identifying patients who could most benefit from earlier, better informed health care interventions. The Sollis commissioning intelligence toolset allows the patient population to be risk assessed to provide timely, evidential data to GPs. This enables GPs to provide focused levels of care to specific groups of patients, reducing the risk of a patient’s condition worsening due to it being identified early.

Sollis Clarity Commissioning software incorporates the Johns Hopkins University’s Adjusted Clinical Groups (ACG®) System which assists clinicians in identifying and improving the care of at-risk patients.  Sollis Clarity Patients integrating ACGs, allows health care professionals to plan future demand by providing real time lists of patients who are at high risk of future hospital admission. The algorithm uses disease codes, primary and secondary care usage, patient demographics and medication prescribing to rank patients into different risk bands. For CCGs the commissioning intelligence delivered provides vital insights into the management of patients with Long term Conditions.
